I recently had the occasion to visit the Oak Ridge Nursing and Rehabilitation Center, located at 261 Terhune Drive, in Wayne, NJ.
Their executive director is Sharon Loganzo.
Oak Ridge is part of the Revera Healthcare System of Skilled Nursing and Rehab facilities

Oak Ridge is a nice facility located in an area I am quite familiar with.
They provide different skilled care services, as well as short term rehabilitation via Premier Therapy Services (a subsidiary of the Revera Healthcare Group)
Why am I familiar with this area?
Well of course it is because the magnificent and 5 Star rated Regency Gardens Nursing and Post-acute Rehabilitation Center is also located in Wayne, NJ.
There aren’t enough adjectives to adequately describe the beauty of the facility and grounds, the exceptional rehab program and the incredibly compassionate care!
You really need to tour the facility and see their sub-acute rehab program (best in New Jersey) and meet their staff, to fully appreciate what I’m telling you.

Our friends at Wellness.com just published my latest (weekly) article entitled “Advanced Directives Checklist.”
I think it’s a great and important read (if I say so myself) and feedback has been very good thus far.

What Are Advanced Directives?
Advanced directives is a written statement of a person’s wishes regarding medical treatment, often including a living will, made to ensure those wishes are carried out should the person be unable to communicate them to a doctor.
Of course, this is often a vital component to consider for seniors living in nursing homes (and their families).
I’ve put together a checklist to aid folks in this process.
I’ve had several occasions to visit the Willow Creek Rehab and Care Center on Easton Avenue in Somerset NJ.
I actually drive right by the Willow Creek facility on my way to the breathtaking Regency Heritage Post-acute Rehabilitation and Nursing Center, located around the corner on DeMott Lane.
The Willow Creek Rehabilitation and Care Center in Somerset, NJ, is a division of the Revera Health Systems and is licensed for 180-beds.

Their services include short-stay rehab, long-term care, and respite and hospice services.
Around the corner on DeMott Lane and nestled against a backdrop of majestic acres of beautifully manicured lawns and colorful plants and flowers, is the 5 Star Rated, World-Class Regency Jewish Heritage Nursing and Rehabilitation Center
A recent multi-million dollar renovation project has transformed Regency Heritage into one of the area’s most elegant and luxurious senior residences. A generous 265-bed capacity addresses both the community’s long- and short-term care needs. Spacious, elegantly appointed rooms, cheery public areas and gracious dining facilities help create an atmosphere of comfort, companionship and community.


Dogcamp, the first pet-friendly nursing home, has opened in Binasco, Milan. It has been developed by the group Segesta and was created to meet the needs of people who are unwilling to abandon or to hand over their dog or cat. The pets are allowed to stay in their owner’s room, which is equipped with all the amenities necessary for the care of pets. The complex also features a green area with trees where the animals can roam freely, as well as a Dog Hotel, consisting of cottages that house the animals when the master is absent, and spaces for pet therapy.

Last night I went to visit a patient at The Manor skilled Nursing and rehabilitation center in Freehold, NJ
This patient is looking to transfer to Regency Park, which is our magnificent Nursing and Post-Acute Rehabilitation Center located in Hazlet, NJ.
The Manor Health and Rehabilitation Center in Freehold, NJ, provides short-term rehabilitation and long-term skilled-nursing services for adults age 18 and older and is located on the campus of CentraState Medical Center. The Manor is part of CentraState Healthcare System’s senior services division.

Late last night I paid a visit to the Roosevelt Care Center at Old Bridge, NJ.
This is a skilled nursing and rehabilitation center located at 1133 Marlboro Rd. Old Bridge NJ 08857.
I was there to meet with a family and a patient who will be transferring today to our magnificent Regency Park Nursing and Rehabilitation Center in Hazlet NJ.
Regency Park is located just a short drive from the Roosevelt Care Center and the family selected us for our world class 5 Star Rated Care.

Roosevelt Care Center is located near the intersection of Routes 9 and 18
The Middlesex County Board of Chosen Freeholders began construction on the 105,000-square-foot center in 2009, and Roosevelt Care Center at Old Bridge officially opened its doors in the fall of 2011. The intent was to extend all of Roosevelt Care’s services to a more convenient location for South County families and residents.

I picked up a fantastic article today by Kevin McMahon, MPA, LNHA, writing for LTL.

Both the acute care and post-acute care (PAC) industries have their full attention focused on the much discussed and hyper-analyzed 30-day readmission metric. On the acute care side, this number determines whether a reimbursement penalty will be applied—affecting revenue streams. On the PAC side, the number increasingly will be used to judge the quality of PAC partners and ultimately communicate that information to discharging patients. High stakes stuff for sure. Unfortunately the fixation on this magical metric might be obscuring various subtle and more nuanced aspects of this readmission puzzle.
As I have considered this metric and its implications both as a PAC administrator and in my current job on the acute care side of the of the healthcare world, I have stumbled on three not-so-obvious angles that PAC providers need to be aware of and, if at issue, develop a means to address.
